Mental Health Self-Assessment

Brief questionnaires can help you reflect on anxiety, depression or trauma-related symptoms and prepare for a conversation with a clinician.

Self-assessment is not a diagnosis. A score cannot confirm or rule out a condition, determine your safety or identify the right treatment. These questionnaires are intended for adults ages 18 and older.

Your answers and scores are not submitted to or monitored by our office. Completing a questionnaire does not request an appointment. Use the scroll area below to view the questionnaires, their results, and the OCD Self-Reflection Guide.

If you cannot stay safe or are in immediate danger, call 911 or go to the nearest hospital emergency department. In the United States, call 988 or text 988 for crisis support. Contact a trusted support person, without delaying emergency help.

Understanding your results

Screening results are a starting point for discussion, not a diagnosis or a recommendation for TMS, PRISM, ketamine or medication changes. A low score does not rule out a condition or mean that you are safe. If symptoms are concerning or affect daily life, consider a clinical evaluation regardless of your score.
