Botulinum toxin injections are best known for cosmetic uses and certain medical conditions. Researchers have also studied injections in the frown-muscle area for major depression. That research should be distinguished from an established, FDA-approved depression treatment.
What has been studied?
One proposed explanation involves feedback between facial expression and emotional processing. This remains a hypothesis rather than a complete explanation of depression or proof that changing an expression will treat it.
In a phase 2 randomized trial published in 2020, 255 adult women received onabotulinumtoxinA or placebo. Results varied by dose and time point. The lower-dose group did not reach conventional statistical significance on the primary depression measure at week six; the higher-dose group did not separate from placebo at that time. Some other results suggested benefit.
These mixed findings support further study, rather than a promise of reliable improvement for an individual patient. One trial also cannot establish how well results apply to other populations or long-term care.
Is Botox approved for depression?
Depression is not among the FDA-approved indications for BOTOX. Using botulinum toxin specifically to treat depression is off-label. Approval for cosmetic lines, migraine or another condition does not establish effectiveness for a psychiatric diagnosis.
What about safety?
Risks depend on the product, injection site and individual health. They can include eyelid drooping and other adverse effects. Product labeling includes a boxed warning about possible spread of toxin effects, with potentially serious swallowing or breathing problems. Off-label use needs an informed discussion of benefits, uncertainty and risks.
