Ketamine for Bipolar Depression: Evidence and Safety

by | Oct 29, 2026 | Patient Guides, Blogs | 0 comments

Depression in bipolar disorder requires a treatment plan that considers both depressive symptoms and the risk of mood elevation. Ketamine has been studied for bipolar depression, but promising early findings do not make it an established treatment for everyone.

Is IV ketamine FDA-approved for bipolar depression?

No. Ketamine injection is approved as an anesthetic. Its use for bipolar depression is off-label. IV ketamine and esketamine nasal spray are different treatments; an approval for one product or diagnosis does not establish approval for the other.

What does the evidence show?

Small studies have reported short-term improvement in some patients. However, study size, certainty of evidence and limited long-term follow-up make lasting benefit and safety difficult to predict.

The 2023 VA/DoD bipolar disorder guideline found insufficient evidence to recommend for or against ketamine or esketamine for acute bipolar depression, either alone or as an added treatment. This describes uncertainty; it is not a recommendation that every patient should receive it.

Why specialist evaluation matters

An evaluation should review the diagnosis, prior depressive and manic or hypomanic episodes, previous treatment, current medicines, substance use and medical history. Bipolar care commonly involves mood-stabilizing medication and may include psychotherapy. Ketamine does not replace an ongoing bipolar treatment plan.

Potential risks include blood-pressure changes, sedation, altered perception and misuse or dependence. Monitoring and a plan for follow-up are essential. Changes such as sharply reduced need for sleep, unusual energy or escalating impulsivity should be reported promptly to the treating clinician.
