Open your paperwork with the link and access code in your email
- Open the PracticeQ email and use the personal form link and access code provided.
- Complete, initial, sign and submit the required forms.
Use your personal PracticeQ invitation to open, complete, sign and submit your paperwork. If you cannot find it, check your spam or junk folder, call 817-659-7344, or contact us through the website to ask for your link to be resent.

Appointment confirmations and reminders
When you consent, our scheduling system sends appointment confirmations and reminders by email and text message. As a courtesy, our office also calls approximately 24 hours before your appointment.
