Depression affects more than mood

Depression can affect energy, sleep, concentration, appetite, relationships, and daily functioning. Some people experience persistent sadness; others feel numb, irritable, or disconnected.

Anhedonia—a reduced ability to experience interest or pleasure—can be particularly difficult. An evaluation helps us understand these symptoms alongside anxiety, trauma, substance use, pain, sleep problems, or medical conditions.

Treatment options

Psychiatric evaluation and medication management

We review your concerns, treatment history, current medications, medical conditions, and goals. Medication may be an appropriate first step or part of a broader plan. We monitor response and side effects and adjust treatment when needed.

Deep TMS

Our practice uses the BrainsWay Deep TMS system. Treatment uses magnetic pulses while you are awake and does not require anesthesia. We assess suitability and safety before recommending it.

TMS does not need to be the last option discussed after years of unsuccessful treatment. Previous antidepressant treatment, the applicable device indication, and individual clinical circumstances guide eligibility.

IV ketamine

For selected patients, IV ketamine may be considered following an evaluation. It can help some people quickly, but improvement is not guaranteed or necessarily lasting. We discuss risks, monitoring, alternatives, and follow-up. Its use for depression is off-label.

PRISM for anhedonia

If loss of interest or pleasure is a central concern, ask about PRISM EEG-based neurofeedback. Its use for anhedonia is investigational. We explain the available evidence, uncertainties, and other options.

How we choose treatment together

We consider your diagnosis, symptom severity, prior treatment response, medical conditions, preferences, and the practical demands of care. Treatments have different risks and requirements; adding more treatments is not automatically better.

Our goal is remission whenever possible, together with improved functioning and a plan to maintain progress. We assess how you are feeling and what you are becoming able to do again.

Clinical experience informed by research

Led by board-certified psychiatrist Diana Ghelber, MD, our team provides individualized psychiatric care. Dr. Ghelber is a coauthor of multicenter studies of Deep TMS outcomes in everyday clinical settings:

These studies contribute to our understanding of treatment; they do not predict an individual outcome or establish eligibility for every age studied.

Supporting your existing psychotherapy

If you already work with a therapist, we can coordinate with your permission. Psychiatric care may help address symptoms that make it difficult to participate fully in that ongoing work.

Feeling stuck in therapy? Depression may be part of the picture.
